That you have read this statement and want the named charity or Community Amateur Sports Club (CASC) to reclaim tax on the donation detailed on this form, given today. That you understand that You must pay an amount of Income Tax and/or Capital Gains Tax in the tax year at least equal to the amount of tax that all the charities and CASCs you donate to, will reclaim on your gifts for that tax year. That you understand that other taxes such as VAT and Council Tax do not qualify. That you understand the charity will reclaim 25p of tax on every £1 that you have given.

"I met John in 1958 when he joined Marconi, We had very good social life including a training run on a Wednesday night. We kept in touch until his death this including his trips to Southend which included meals out. You will be greatly missed my friend."

"I got to know John when I started training at BH Live Active Pelhams about eight years ago. His knowledge of the fitness industry was immense. He was a gentleman, always had time for you and we shared a great many stories. I worked as his gardener and it was a pleasure working on his garden, which was at its peak in the spring. My wife and sons came to know him too and he will be greatly missed."
