Due to the current situation, a Memorial Service for close family and friends followed by an Interment of Ashes Ceremony in the Harbour View Woodland Burial Ground in Poole, Dorset, UK, will take place later in the spring or early summer.

Important Note

That you have read this statement and want the named charity or Community Amateur Sports Club (CASC) to reclaim tax on the donation detailed on this form, given today.
